Support Coordination is carried out together with and on behalf of participants. It involves identifying, coordinating and accessing supports and services that meet your specific needs, wants and preferences.
This is one of the key PWS supports we provide in Sydney and surrounding areas
How does Support Coordination help?
Support Coordination is a service that matches participants with a dedicated Support Coordinator that helps to:
Explain what is and isn’t covered in an NDIS plan
Plan the best way to use NDIS funding
Connect participants with different community services and supports
Prepare NDIS reviews
Support Coordinators offer choice and control around the types of supports you will need. Once you have secured your NDIS funding, you can begin the process of finding the different supports you will need to meet your goals.
To make sure you are able to carefully navigate the various PWS NDIS supports, we strongly recommend that you ask for Support Coordination when first applying for funding.
